Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.     On or about June 1, 2012, employees were working on a service rig which proper procedures such as locking the draw works brake system were no implimented.  This conditon exposed employes to stuck by hazards.     Referencing API RP 54 9.4.4: The equipment operator shall not leave the draw works brake without tying down the brake or securing it with a catch lock, unless the draw works is equipped with an automatic driller.
