29 CFR 1904.4(a)(1):  Each employer required by this part to keep records of fatalities, injuries, and illnesses did not record each fatality, injury, and illness that is work-related:    (a)	The Lakeside Association, 236 Maple Avenue, Lakeside, Ohio:  On or about June 5, 2012, at the worksite located at Hotel Lakeside, 150 Maple Avenue, Lakeside, Ohio, the employer did not ensure that an OSHA 300 form or equivalent form was maintained for calendar year 2012.  An employee was injured resulting in approximately 95 days of restricted duty and the injury was not recorded on the OSHA 300 form

29 CFR 1910.141(a)(5):  Every enclosed workplace was not constructed, equipped, and maintained, so far as reasonably practicable, as to prevent the entrance or harborage of rodents, insects, and other vermin.  A continuing and effective extermination program was not instituted where their presence was detected.      (a)	 The Lakeside Association, 236 Maple Avenue, Lakeside, Ohio:  On or about June 5, 2012, at the worksite located at Hotel Lakeside, 150 Maple Avenue, Lakeside, Ohio, the employer did not ensure that the vermin control program effectively prevented wild animals from entering the building.
