29 CFR 1910.134(e)(1):   The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:      (a)    Employees were required to wear the 3M negative pressure, half face-piece respirator fitted with P95 particulate filters and have not had a medical evaluation.

29 CFR 1926.451(b)(1)(i):   Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary:    (a)    The work levels at the center section and the North section of the three section rolling scaffold had only one fabricated plank installed and were not fully planked across the entire work level.

29 CFR 1926.451(g)(1)(vii):   Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:      (a)    The work level of the three section rolling scaffold was at 12 feet height and had no guard rails.

29 CFR 1926.452(w)(1):   The mobile scaffold was not braced by cross, horizontal, or diagonal braces, or combination thereof, to prevent racking or collapse of the scaffold and secure vertical members together laterally so as to automatically square and align the vertical members:    (a)    The center section of the three section rolling scaffold had three fabricated planks installed at three different heights but had no cross braces, horizontal braces, or diagonal braces installed.      (b)    There were missing cross braces on the North and South sections of the three section rolling scaffold.
