(a) Location - Florence Street, Cambridge, MA:  On or about 4/2/12, Scaffolding was not erected under the direction of a competent person. Scaffold was not level, not secured properly and no baseplates were installed.

(a) Location - Florence Street, Cambrige, MA:  On or about 4/2/12, employees were not protected from falling while working from a scaffold that was greater than 10 feet above a lower level.

(a) Location - Florence Street, Cambridge, MA:  On or about 4/2/12, employees were not protected from falling while working on a roof and second floor greater than six feet above a lower level.
