29 CFR 1926.451(c)(2)(iv):  Front-end loaders and similar pieces of equipment were used to support scaffold platforms and were not specifically designed by their manufacturer for such use:    (a)  Carvell Welding, LLC, 350 South Main Street, Manheim, PA - A sixteen (16) foot x five (5) foot unguarded work platform was placed on the elevated forks of a Pettibone (Model 8044) telehandler where an employee was standing and cutting down metal beams.  Pettibone does not permit the use of work platforms for elevating personnel on the forks of its telehandlers, on or about May 24, 2012.    Abatement certification required within 10 days after abatement date.   The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

An operator was not present at the controls, thus exposing the employee to injury, on or about May 24, 2012.    Abatement certification required within 10 days after abatement date.   The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
