Florence Avenue, Toledo, OH: the employer did not ensure that a personnel platform used to lift employees and materials to the roof was secured to the lifting carriage of the rough terrain fork lift.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) Leupp Building Services, Inc., on or about June 6, 2012, at work site located at 202 W. Florence Avenue, Toledo, OH: the employer did not ensure employees were protected from fall hazards while riding on the platform to perform work to the edge of the roof. Employees were exposed to a 12 foot fall hazard in that no front or side guardrails or fall protection were used while employees were lifted by the Gehl (model #513DM54, SN# E071107) rough terrain fork lift.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. a) Leupp Building Services, Inc., on or about June 6, 2012, at work site located at 202 W. Florence Avenue, Toledo, OH: the employer failed to assure employees were protected from a fall up to 20 feet to the ground below during steel erection work. Employees were exposed to fall hazards while removing and installing metal decking.
