At the time of the inspection the electrical extension cords providing power to two air compressors did not have a ground pin installed. Employees were exposed to shock hazards.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage.  At the time of the inspection a yellow AWG 12/3 extension cord was run across the road way were it was being runover by construction equipment. Employees were exposed to a shock hazard.

The two yellow AWG 12 extension cords being used to provide power to the air compressors being used for siding installation from metal scaffolds had several areas where the outer insulation was broken/worn away. Employees were exposed to a shock hazard.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.  At the time of the inspection the scaffolds being utilized had not been inspected by a competent person to ensure the scaffolds in use were properly errected. Employees were exposed to falls of approximately fifteen (15) feet to the compacted ground below.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  At the time of the inspection employees were observed to be performing siding operations from jack-up type scaffolds that did not have fall protection installed on the ends and only had an upper railing on the remainder of the scaffold to act as fall protection. Employees were exposed to a fall hazard in excess of fifteen 915) feet to the ground below.

At the time of the inspection employees were observed to be performing siding operations on juck-up scaffolds and these employees had not been trained to recognize and abate the hazards.
