29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. On or about 05/07/2012, the employer did not ensure that point of operation/chuck guards were installed on all lathes located in the shop area of the facility to protect employees from chuck components that may loosen and disengage from the chuck. The following lathes are used by employees and are missing chuck guards: Instances: a. Clause 8 inch horizontal lathe b. Grizzly G0509a Horizontal lathe, s/n 030937 c. Monarch large horizontal lathe #15267

29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed. On or about 05/07/2012. the employer did not ensure that open holes in circuit breaker panels were not effectively closed. Circuit breaker panels C, C1 & D had open holes on the iinside dead face where circuit breakers had been removed, which could potentially be an electric shock hazard.
