29 CFR 1926.501(b)(4)(iii): Each employee on a walking/working surface was not protected from objects falling through holes (including skylights) by covers: (a) Henderson Biobehavioal Health Building Third Floor- Room 310 B: A floor hole, twelve and one half inches long by six inches wide, used as a temporary electrical chase, was not covered to prevent material from falling to the second floor and striking employees, on or about May 11, 2012. No abatement certification or documentation required.
