29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems or personal fall arrest systems. Employees applying insulation to the roof of the building under construction did not have fall protection in place while hoisting up materials. The employees were exposed to a 13-foot to 32 -foot fall to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. At this job site, employees were not trained in the following: a. Employees who were required to work on a roof at various heights were not trained in the proper selection, use and maintenance of the fall protection system provided. b. The employer had not designatd and trained a person to be qualified person for installation of fall protection systems.
