The certification shall be made available to each employee entering the space or to that employee's authorized representative.    a)  Workplace, 47-49 Old Lincoln Highway, Kearny, NJ     an employee entered a tank to retrieve a plastic cutter device that fell in the tank.  The employer did not document that all hazards were eliminated.     Violation occurred on or about 5-1-12    b)  Workplace, 47-49 Old Lincoln Highway, Kearny, NJ     an employee entered a tank to wash an interior stain with a cleaning solution.  The employer did not document that all hazards were eliminated.     Violation occurred on or about 5-27-12

29 CFR 1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a) Workplace, 47-49 Old Lincoln Highway, Kearny, NJ     The employer did not develop an energy control program for an employee who performed service on water pressure pumps, and had performed service on water pressure pump # 15, removing the pump for service then replacing it with the repaired pump.  The pumps are powered by electric motors that operate with 240 Volts and approximately 20 amps of power.     Violation determined on or about 6-5-12
