29 CFR 1910.1200(e)(1):  The employer did not develop, implement, and maintain at each workplace a written hazard communication program which at least described how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met:    a)  At the facility - A written hazard communication program had not been developed, implemented and maintained.  Employees use hazardous chemicals such as, but not limited to, Portland Cement (a corrosive) and Duogard Form Release Agent.

29 CFR 1910.138(b): Employers did not base the selection of the appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the task(s) to be performed, conditions present, duration of use, and the hazards and potential hazards identified:    a) In the facility, on or about June 1, 2012 -  Employees used cotton gloves while handling Portland cement and form oil.  The cotton gloves had to be changed frequently due to saturation with cement.
