29 CFR 1926.651(j)(2):     Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.    a) Vaught/Hemmingway Stadium, University of Mississippi, Oxford, MS  On or about June 6, 2012, two employees were in an excavation 84 inches deep without any protection from the spoil pile falling into the excavation because it was at the edge of the opening.    b) Vaught/Hemmingway Stadium, University of Mississippi, Oxford, MS  On or about June 6, 2012, two employees were in an excavation 84 inches deep without any protection from the backhoe outrigger falling into the excavation because one of the outriggers was on the edge of the opening.

29 CFR 1926.652(b):     The slopes and configurations of sloping and benching systems were not in accordance with the requirement options of 29 CFR 1926.652(b)(1), (b)(2), (b)(3), or (b)(4):    a) Vaught/Hemmingway Stadium, University of Mississippi, Oxford, MS  On or about June 6, 2012, two employees were in an excavation 84 inches deep without any protection from cave-in because the slope of two walls were .657 to 1 and .728 to 1. The sample results were soil type B which requires a slope of 1 to 1.
