29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section:    The employer failed to develop specific energy control procedures for production equipment including but not limited to the M&R Cayenne Screen Printing machines . Employees would perform lockout tagout based on their knowledge and experience. Lockout tagout was performed when servicing and maintaining equipment.       Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.178(l)(6): The employer did not certify that each operator had been trained and evaluated as required by this paragraph (l)    Employer failed to certify that each authorized forklift operator had been trained and evaluated every three years. Employees were operating powered industrial vehicles to load and unload materials received and to transport material to the different workstations.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
