29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) Back south corner of building: A guard was not in place on the front of the CBBY Ajax body press machine model RCBBY serial # ND166913 where the garment was loaded onto the press, on or about 6/1/12. b) Back south corner of building: A two hand pushbutton control guard on the Ajax collar and cuff press was not used properly when two employees operated the machine simultaneously, on or about 6/1/12. c) Back northeast corner of building next to boiler room: A guard was not in place over the point of operation on a Chandler Button Master sewing machine model 8524, on or about 6/1/12. d) Center of building behind the dry cleaning machines: A guard on the Forenta and Ajax dry cleaning presses was not in place to ensure employees removed their hands before closing the lids, on or about 6/1/12. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
