29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): a) Employees were exposed to the hazard of falling, approximately 20 feet to the ground, while performing masonry work from tubular welded frame scaffold platform that was not fully planked. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a) Employees working from tubular welded frame scaffolding were not provided with proper access to a work platform being used at an elevation greater than 25 feet above the ground. Edward Cross and Son, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1) which was contained in OSHA violation number 312403991, citation 1, item 1, issued on September 08, 2008 and became final order on or about September 15, 2008 with respect to a workplace located at 1026 Park, River Forest, IL 60305. No abatement certification or documentation is required for this item.
