29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) A Hazardous Energy Control Program was not developed to ensure that employee(s) who perform maintenance and repair on production equipment, such as but is not limited to CNC machines, Denison Press, milling machines, and air compressor, are not exposed to unexpected energizing, startup or release of stored energy from mechanical and electrical components.

29 CFR 1910.147(c)(4)(i):     Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    (a) Written step by step procedures were not developed for authorized personnel to follow when accomplishing maintenance or servicing work on, but not limited to the Didion sand system, shakers, molders, melting pot systems, and grinders. This exposed personnel to caught-in, crushing and amputation hazards.

29 CFR 1910.212(a)(3)(ii):     Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    (a) Denison Multi Press #14: Employees operate the Denison hydraulic press without having the point of operation guarded, exposing the employee(s) to an amputation hazard.    (b) Milwaukee Simplex Milling Machine: Employee(s) operate the Simplex milling machine without having the point of operation and the milling blade guarded to prevent employee(s) from coming into contact with the point of operation and rotating cutting blade. This exposed the operator to an amputation hazard.
