29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening: (a) On or about 6/6/12, in the receiving area, a ladder leading to a work platform was not guarded with a chain or swinging gate to prevent employees climbing this ladder from falling. The employee using this ladder was exposed to fall hazards of approximately 11 feet to the concrete below.

29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: (a) On or about 6/6/12, in the Mixing Room, the work platform at the top of the stairway leading to the grain chute had a hole in it, exposing employees to tripping hazards.
