29 CFR 1910.22(b)(1): Where mechanical handling equipment is used, sufficient safe clearances were not allowed for aisles, a loading docks, through doorways and wherever turns or passages must be made: On or about June 7, 2012, when the ingots are taken to the scales by overhead crane, they are taken off of the hook and are left standing on end while being weighed. Adequate safe clearance for the nearby passageways was not provided as the ingots have fallen directly into these passageways, nearly striking the workers in, around, or passing by the weighing process.

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points: On or about June 7, 2012, in the melt shop, the weld table had a wheel with a broken bearing where workers would have to place shims between the ingot and the wheel to level the ingot. The workers are exposed to pinch or crushing hazards due to no guarding in place to prevent contact with an ingoing nip point.
