29 CFR 1910.305(b)(1)(ii):     Unused openings in boxes, cabinets, or fittings were not effectively closed:    a) Breaker Panel Shop Wall; On or about June 7, 2012 and at times prior thereto the employer exposed employees to electrical hazards in that employees were allowed to access and work around a breaker panel box with an open breaker slot exposing a live 220 volt AC buss bars inside the electrical panel.

29 CFR 1910.1200(e)(1):     The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    a) Shop Floor; On or about June 7, 2012 and at times prior thereto the employer exposed employees to hazardous chemicals and substances that include but are not limited to hydraulic oil, motor oil, transmission fluid, grease, diesel, gasoline, coolant, degreaser, brake cleaner, and starter fluid without maintaining a written hazard communication program, MSDS's, and providing the employees with information and training on chemicals and substances as required by the regulation.

29 CFR 1910.1200(h)(1):     Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets.    a) Shop Floor; On or about June 7, 2012 and at times prior thereto the employer exposed employees to hazardous chemicals and substances that include but are not limited to hydraulic oil, motor oil, transmission fluid, grease, diesel, gasoline, coolant, degreaser, brake cleaner, and starter fluid without providing the employees with information and training on chemicals and substances as required by the regulation.
