29 CFR 1926.453(b)(2)(v): A lanyard was not attached to the boom or basket when working from an aerial lift: Employees working from the TB85 boom lift located at 360 E. William St, Wichita, KS 67202 were exposed to fall hazards of approximately 24 feet to the ground below in that, the lanyard used by the employee for fall protection was not attached to an anchorage point while the employee was in the basket.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: Employees working from the first deck level of the parking garagae under construction at 360 E. William St, Wichita, KS 67202 were exposed of fall hazards of 14' to 19' in that, fall protection was not utilized near unprotected sides and edges.
