29 CFR 1910.184(d): Inspections. Each day before being used, the sling and all fastenings and attachments shall be inspected for damage or defects by a competent person designated by the employer. Additional inspections shall be performed during sling use, where service conditions warrant. Damaged or defective slings shall be immediately removed from service.    On or about 05/15/2012, the employer did not ensure that all synthethic slings used at the facility were inspected to detect damage. A 1 inch synthetic sling was being used to move sections of pipe in the pipe shop which was severely frayed and cut in numerous locations. This potentially exposes employees to being struck by the pipe should the sling break.

29 CFR 1910.253(b)(5)(iii)(B): Cylinders shall be handled carefully. Rough handling, knocks, or falls are liable to damage the cylinder, valve or safety devices and cause leakage.    On or about 05/15/2012, the employer did not ensure that fuel gas cylinders that were located in the shop were not secured to prevent them from falling over and damaging and/or breaking the valve. This exposed employees to being struck by a projectile should the valve break off.    INSTANCES:        1.  2 Argon cylinders were located in the aluminum shop and were unsecured and located in a travelled area        2.  An Acetylene cylinder was located near the inner wall of the "Back" shop and was unsecured with forklifts operating in            the area.

29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed.    On or about 05/15/2012, the employer did not ensure that open holes in circuit breaker panels were closed. The large circuit breaker panel located in the tube & pipe shop had openings where circuit breakers were removed and covers had not been installed, exposing employees to live circuits, and to dust/moisture depositing on contacts creating shorting conditions..
