29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: (a) On or about 6/5/12, in the parking lot on the east side of the building, the employees were working in the basket of the telescoping boom lift and were not attached to the boom or basket. The employees were exposed to fall hazards of approximately 30 feet to the concrete below.

29 CFR 1926.453(b)(2)(vi): Boom and basket load limits specified by the manufacturer were exceeded: (a) On or about 6/5/12, in the parking lot on the east side of the building, two employees were working in the basket of the telescoping boom lift and exceeded the load limit specified by the manufacturer. The employees were exposed to fall hazards of approximately 30 feet to the concrete below.
