This created a condition which exposed employee(s) to the hazard(s) of eye irritation.    Instance b:  On or about June 8, 2012, at the polishing room/sanding department, where the employer did not ensure that affected employees required to wear safety spectacles with unperforated sideshields while sanding and polishing wood cabinetry components were protected from flying wood and silica dust particles . This created a condition which exposed employee(s) to the hazard(s) of eye irritation.
