29 CFR 1910.23(a)(1): Stairway floor opening(s) were not guarded by a standard railing on all exposed sides (except at the entrance to opening) that were constructed in accordance with paragraph (e) of this section: (a) General Mills Inc., S Grain Elevator, Superior, Wisconsin; Employees were exposed to fall hazards of 15 feet or greater at multiple spiral stairway floor openings on the top levels of the S Elevator because the guard rail system did not have an intermediate rail approximately halfway between the top rail and floor.

29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program which at included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii): (a) General Mills S Elevator, Superior, Wisconsin: The employer did not implement and maintain a written hazardous communication program where employees were potentially exposed to chemical such as, but not limited to, Aluminum Phosphide used in the workplace as a grain handling fumigant.
