Jobsite located at 300 Richmond Street, Providence, RI, Rooftop: On or about 06/06/2012, a warning line was not erected to connect the work area to the materials handling area and points of access by an access path formed by two warning lines in the following instances: (a) Point of access Elm Street , first level,: a warning line was not installed from the ladder access point to the second level access point approximately 25 feet away; (b) Point of access second level: a warning line was not installed from the ladder access to the work area; and (c) Materials handling area a warning line was not installed from the work area to the material storage and handling area.

29 CFR 1926.502(h)(1)(i): The safety monitor shall be competent to recognize fall hazards: Jobsite located at 300 Richmond Street, Providence, RI, Rooftop: On or about 06/06/2012 the Safety Monitor did not recognize and correct the hazardous conditions regarding the fall protection system being used in the following instances: a) Sections of the warning were missing flags and the flags were faded faded and no longer retained their high-visibility appearance and the multiple sections were missing flags; b) The warning line was incomplete and did not enclose the work area, extend to include the material storage area, and did not extend to the ladder access areas; c) The designated Safety Monitor was engaged in work activities; and d) Employees were allowed to conduct work activities in unprotected areas of the roof.
