29 CFR 1910.141(g)(2): Eating and drinking areas. No employee shall be allowed to consume food or beverages in a toilet room nor in any area exposed to a toxic material.    On or about 06/01/2012, the employer did not ensure that areas where shop employees ate lunch and took breaks were clean and sanitary and free of any toxic material contamination. Employees were observed consuming food and beverages at tables set up in the shop area adjacent to where welding machines were located and used.

29 CFR 1910.215(a)(4): Work rests. On offhand grinding machines, work rests shall be used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion.    On or about 06/01/2012, the employer did not ensure that the Baldor bench grinder located next to the welding machines, had the work rest properly adjusted. The work rest was 0.75" away from the face of the abrasive wheel. When in use work or tools can be drawn into the wheel, breaking the wheel or becoming a projectile.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage shall be separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.    On or about 06/01/2012, the employer did not ensure that oxygen and fuel gas cylinders were not stored together. Oxygen and Acetylene cylinder were stored in a metal enclosure outside the shop building next the the property line on the east side. The cylinders were less than 4 ft apart.

29 CFR 1910.303(g)(1): Space about electric equipment. Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.  On or about 06/01/2012, the employer did not ensure that easy access to circuit btreaker panels was provided. Access to circuit breaker panels in the locations below was blocked prebventing employees from getting to the panels in an emergency.  INSTANCES: a. A circuit breaker panel located behind the Baldor bench grinder inside the shop area, was blocked by the bench grinder b. 2 circuit breaker panels in the SW corner of the open work area were blocked by large pieces of material.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidentalcontact by use of approved cabinets or other forms of approved enclosures or by any of the following means:  On or about 06/01/2012, the employer did not ensure that exposed electric wiring. conductors, etc were not protected from being contacted by employees. The circuit breaker panel located on the north wall of the open work area was missing the dead front which exposed live electric circuits to contact.

29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed.    On or about 06/01/2012, the employer did not ensure that all openings in the dead front of the circuit breaker panel on the North wall of the open work area were effectively closed. Numerous openings were observed when the dead front was re-installed. The openings where circuit breakers had been removed, allow for dust and moisture to deposit on the live electrical wiring, conductors, etc.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.    On or about 06/01/2012, the employer did not ensure that extension cords used in the shop area were in good and useable condition. A yellow extension cord and a black extension cord were attached to wall outlets behind the threading machine along the north wall of the inside shop area. Strain had been applied to both cords which permitted the insulation to be pulled back from the plug ends and exposing the internal conductors and creating potential electrci shock hazards.
