29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    On or about 05/14/2012, the employer did not ensure that employees that were required to work from elevations were trained in fall protection. Employee(s) were working on a mezzanine with unguarded sides at an elevation of 9 ft above a cement floor without fall protection.

29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about 05/14/2012, the employer did not ensure that employees renovating an old building were protected from falling when working at high elevations. Employees were not wearing any type of personal fall protection when removing expamded metal grating from the mezzanine level which was 9 feet above the cement floor below.    Airport Road Welding Service was previously for a violation  of thos Occupational Safety and  Health standard, which was contained  in OSHA inspection number 314768227, citation number 1, item number 1, and was affirmed as a final order on 03/10/2011, with respect to a workplace ;ocated at: W Thomas St, Hammond LA.

29 CFR 1910.134(c)(2)(i): An employer may provide respirators at the request of employees or permit employees to use their own respirators, if the employer determines that such respirator use will not in itself create a hazard. If the employer determines that any voluntary respirator use is permissible, the employer shall provide the respirator users with the information contained in Appendix D to this section ("Information for Employees Using Respirators When Not Required Under the Standard"); and    On or about 05/17/2012, the employer did not provide employees using their own respirators on the worksite with a copy of Appendix D of 29 CFR 1910.134

29 CFR 1910.134(c)(2)(ii): In addition, the employer must establish and implement those elements of a written respiratory protection program necessary to ensure that any employee using a respirator voluntarily is medically able to use that respirator, and that the respirator is cleaned, stored, and maintained so that its use does not present a health hazard to the user. Exception: Employers are not required to include in a written respiratory protection program those employees whose only use of respirators involves the voluntary use of filtering facepieces (dust masks).    On or about 05/17/2012, the employer did not develop & implement a written respiratory protection program for employee(s) voluntarilly using personal respirator(s) that were engaged in renovation operations. An employee was wearing a personally owned 1/2 face respirator at the time of the incident.
