29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports as follows:    a.    On the northside of  Building B where an employee was observed working on a tuular welded frame scaffold, the general contractor did not ensure that the working level of the scvaffold was fully planked thereby exposing the employee of Vibo Construction to a fall hazard.

29 CFR 1926.451(g)(4)(i):     Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.    a.    On Building A, Southeast side of the 3rd level of the scaffold where an employee was unloading brick from the pallet on the end of a rough terrain forklift, the general contractor did not ensure a guardrail was installed thereby exposing employees of Vibo Construction to a 20 foot fall hazard.    b.    On the west scaffold,  northwest side of the second level where employees of Vibo Construction were laying bricks, the general contractor did not ensure guardrails were installed thereby exposing employees to a 11'6" fall hazard.

29 CFR 1926.502(b)(2):     Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structural members shall be installed between the top edge of the guardrail system and the walking/working surface when there is no wall or parapet wall at least 21 inches (53 cm) high.    a.    On the second floor of Building A, there was a floor hole where the plywood was knocked out to install a trash chute and the opening did not have a midrail thereby exposing employees to a fall hazard.

29 CFR 1926.1052(c)(1)(i):     At least one handrail; and:    29 CFR 1052(c)(1)(ii):     One stairrail system along each unprotected side or edge.  Note: When the top edge of a stairrail system also serves as a handrail, paragraph (c)(7) of this section applies.    a.     In Building A where framing work was performed, the stairways from the first to the third floor were not provided with hand rails thereby expoosing employees to a fall hazard.
