On Building A, Southeast side of the 3rd level of the scaffold where an employee was unloading brick from the pallet on the end of a rough terrain forklift, the general contractor did not ensure a guardrail was installed thereby exposing employees of Vibo Construction to a 20 foot fall hazard.    b.    On the west scaffold,  northwest side of the second level where employees of Vibo Construction were laying bricks, the general contractor did not ensure guardrails were installed thereby exposing employees to a 11'6" fall hazard.
