29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about June 6, 2012, in the interior of the building, employees using a hand held electric drill did not use eye protection, exposing themselves to eye injuries from flying metal debris.

29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of American National Standards Institute, B7.1-1970, Safety Code for the Use, Care and Protection of Abrasive Wheels, and paragraph (d) of this section: On or about June 6, 2012, in the interior of the building, employees were using a handheld grinder that was missing the guard, exposing themselves to being struck by the rotating element of the grinder wheel.
