29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports such that the space between adjacent units and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary (for example, to fit around uprights when side brackets are used to extend the width of the platform):      a. On or about June 7, 2012, on the north side of the building, scaffold number 1 was not fully planked, in that the spaces between the planks were 18-inches, exposing employees to a 13-foot fall hazard.    b. On or about June 7, 2012, on the north side of the building, scaffold number 2 was not fully planked, in that the spaces between the planks were 18-inches,  exposing employees to a 13-foot fall hazard.     c. On or about June 7, 2012, on the north side of the building, scaffold number 3 was not fully planked, in that the spaces between the planks were 18-inches,exposing employees to a 13-foot fall hazard.

On or about June 7, 2012, on the north side of the building, scaffold number 1was planked with one 9-inch wooden plank, exposing employees to a 13-foot fall hazard.    b. On or about June 7, 2012, on the north side of the building, scaffold number 2 was planked with one 14-inch aluminum expandable plank, exposing employees to a 13-foot fall hazard.

29 CFR 1926.451(e)(1):  When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross-braces were used as a means of access:      a. On or about June 7, 2012, on the north side of the building, on scaffold number 2, employees used the cross members to access and egress the scaffold, exposing themselves to a 13-foot fall hazard.
