29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    a) South, west and north sides of the 1st and 2nd levels; On or about June 6, 2012 and at times prior thereto, employees installing lathe and prepping for stucco were working on 3 foot wide tubular welded frame scaffold that had sections that were not fully planked. Only two planks were used in lieu of the three that were needed.    Note: Abatement certification is Not required for this item.

29 CFR 1926.451(b)(3)(ii):     The maximum distance from the face for plastering and lathing operations exceeded18 inches (46 cm).    a) South, west and north sides of the 1st and 2nd levels; On or about June 6, 2012 and at times prior thereto, employees installing lathe and prepping for stucco were working on 3 foot wide tubular welded frame scaffold that had sections where the platform ranged up to 28 inches away from the face of the structure.
