29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous. On or about June 8, 2012, on the east side of a building, employees utilizing a 12 inch power source adaptor with a missing ground pin to energize portable skill saws were exposed to electrical shock hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On or about June 8, 2012, on the east side of the building, employees utilizing an extension cord that was not secured to devices to prevent pull from the terminal screws were exposed to the hazard of electrical shock.
