29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it was infeasible or that it created a greater hazard to use these systems, and did not develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a)  G M Construction at 484 Wind River Drive, Windsor, CO 80550:  On or before June 9, 2012, the employer did not ensure that employees were protected from falling in the following instances:    1)  Employees were framing near an open stairway hole to the basement, and no guardrails were in place, and no personal fall protective equipment was in use.  The stairway hole measured 40" x 134" and was 106' deep;    2)  Employees were sheathing the second floor landing, about 25' x 35', and no guardrails, personal fall arrest system, or fall monitoring protection was used; and    3)  Employees were framing near an open basement window well opening, and no cover or guardrails were in place.  The window well hole 37" x 51" and was 51" deep.    Condition exposed employees to fall hazards of up to 18 feet from the second floor to the basement.    Abatement note:  Abatement certification is required for this item (See enclosed "Certification of Corrective Action Worksheet").
