29 CFR 1926.451(c)(1)(i): Guys, ties, and braces were not installed at locations where horizontal members support both inner and outer legs. a) Chipotle Grill, Worksite - On or about May 11, 2012, employees were working from a tubular welded frame scaffold that was missing crossbracing, at no less than four inner locations, that did not provide the correct support required to allow for additional sections to be built upon them.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. a) Chipotle Grill, Worksite - On or about May 11, 2012, employees were accessing and egress a tubular welded frame scaffold by climbing crossbracing or end frames, from points no less than 13 feet above grade, that do not meet the definition of a standard ladder.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. a) Chipotle Grill, Worksite - On or about May 11, 2012, employees were marking out areas for the installation of exterior insulation and finishing system materials along a scaffolding platform and were not protected from a fall hazard of no less than 13 feet above finished grade by a standard guardrail or other means of fall protection.
