29 CFR 1910.120(q)(6)(i)(C): First responder awareness level. First responders at the awareness level are individuals who are likely to witness or discover a hazardous substance release and who have been trained to initiate an emergency response sequence by notifying the proper authorities of the release. They would take no further action beyond notifying the authorities of the release. First responders at the awareness level did not have sufficient training or experience to objectively demonstrate competency in the following areas: (a) The ability to recognize the presence of hazardous substances in an emergency. Employees did not recognize the presence of damaged asbestos insultation at the Rolling Mill # 5 Table on or about May 13 and 14, 2012. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.1001(j)(4)(i): Labeling. Warning labels were not affixed to all raw materials, mixtures, scrap, waste, debris, and other products containing asbestos fibers, or to their containers. When a building owner or employer identified previously installed ACM and/or PACM, labels or signs were not affixed or posted so that employees were be notified of what materials contain ACM and/or PACM. The employer did not attach such labels in areas where they were clearly be noticed by employees who were likely to be exposed, such as at the entrance to mechanical room/areas. Signs required by paragraph (j)(3) of this section were not being posted in lieu of labels which contained information required for labeling: Rolling Mill #5 Table - The employer did not ensure asbestos containing pipe insulation was labeled or signage required by 29 CFR 1910.1001(j)(3) were posted in that they were not located at the entrances to the building which employees used. The signs that were posted did not comply with the terms of the AISI Settlement agreement in that the sign that were posted did not contain the specific information listed in the settlement agreement including that all thermal system insulation and sprayed or troweled-on surfacing material at the facility must be treated as asbestos unless the material is known or determined not to contain asbestos. The signs also shall identify all other generic types of potential asbestos-containing materials in the facility, such as flooring, roofing, siding, valve packing and gaskets, and electrical insulation and mounting materials, and state that such materials must be treated as asbestos unless the material is known or determined not to contain asbestos. Additionally, such signs were not posted where employees (including contractor employees) who may disturb such components or materials can see the signs on a regular and routine basis in that the sign posted outside the roller mill Table 5 area were posted at a door that was not routinely used by employees and was deteriorated.
