29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)    a)  jobsite, 1121 French Street in Erie, PA, east side of building.  On or about June 8, 2012, personnel were on a tubular welded frame scaffold performing stucco operations and related work.   Platforms of the scaffold, where employees were working, were not fully planked or decked.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used:    a)  jobsite, 1121 French Street in Erie, PA.  On or about June 8, 2012, personnel were on a tubular welded frame scaffold performing stucco operations and related work. Ladders or another acceptable means to access the scaffold, 3 bucks high (20 feet), was not used.  The workers used the side bars and lower platforms to ascend and descend the scaffold.

29 CFR 1926.451(g)(1):     Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a)  jobsite, 1121 French Street in Erie, PA.  On or about June 8, 2012, personnel were on a tubular welded frame scaffold performing stucco operations and related work.  Employees working on the scaffold were not protected from falling 20 feet to the lower level.  Personnel were not protected from falling by properly erected standard railings and/or personal fall arrest systems.

On or about June 8, 2012, employees were on a tubular welded frame scaffold performing stucco operations and related work. Employees working on the lower scaffold platforms and on the ground were not wearing hard hats to protect them from being struck by tools, materials or equipment falling from the scaffold.

On or about June 8, 2012, personnel were on a tubular welded frame scaffold performing stucco operations and related work. Employees working on the lower platforms and on the ground were not protected from being struck by tools, materials or equipment falling from the scaffold. Falling object protection was not installed and utilized on, around, and or with the scaffold.

On or about June 8, 2012, and times prior thereto, Pritchett Construction did not initiate and maintain safety and health programs, in that the company did not provide its employees with general safety and health rules and training when working on the construction site.
