OSH ACT of 1970 Section (5)(a)(1):     Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing for likely to cause death or serious physical harm to employees in that employees were exposed to:    a) Advanced Cast Products, Inc. / 18700 Mill Street, Meadville, PA  Foundry Area: On or about June 7, 2012, employees were exposed to burn hazards from molten metal reacting with moisture in the refractory material and spewing into the work environment.  Employee received burns from a reaction that caused molten metal to spew out of the ladle while it was being transported by forklift to an Autopour.  The refractory material for the ladle was not installed in accordance with prescribed methods in that the refractory mix was estimated (note measured), water for the refractory mix was estimated (not measured), temperature measurements were not taken during curing, and drying/curing time was estimated.

29 CFR 1910.132(a):     Protective equipment was not used whenever hazards capable of causing injury and impairment were encountered.    a) Advanced Cast Products, Inc. / 18700 Mill Street, Meadville, PA: On or about June 8, 2012, employees in the foundry department were potentially exposed to burn hazards from molten metal as it was being transported from the furnace to an autopour machine.  Personnel performing activities that included but were not limited to walking through the foundry area and completing general labor tasks did not wear/utilize protective clothing designed to prevent thermal burns to the torso, arms, and legs.
