This violation was discovered on June 04, 2012, where the employer did not develop, document, and utilize procedures that clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy when employees serviced and maintained equipment and machinery.

This violation was discovered on June 6, 2012, in the maintenance shop; where employees operated a Delta drill press that did not have a shield and chuck guard.

29 CFR 1910.215(b)(9): The distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top exceeded one-fourth inch:    The employer does not ensure that the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top does not exceed one-fourth inch.  This violation occurred on June 06, 2012, in the maintenance shop; where at least two (2) employees used a bench grinder that did not have the adjustable tongue guard in use and adjusted to one-fourth inch of the wheel periphery.
