29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Trash Room Roof, West Side of the Building - An extension ladder did not extend 3' past the landing. A foreman and roofers were observed using a ladder which only extended 30" past the top of the parapet wall exposing them to a 16 fall hazard.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety net s a) Trash Room Roof, West Side of the Building - A foreman and roofers were observed without fall protection while on the roof exposing them to a 16 fall hazard. Built Up Roofing Systems was previously cited for a violation of this occupational safety and health standard, or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA Inspection # 312807399, Citation # 1, Item # 1, and was affirmed on 04/23/09, with respect to a workplace located at 3819 Jay Street, Bldg. 17, NE, Washington, DC 20019.
