29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control (a) At the concrete plant, building G, the machine such as but not limited to Hawkeye machine #1, Model 6X8 H.O. Pipe Plus S/N PP-161, and the procedures to lock out/tag out the machine missed: " the techniques to be utilized for the control of hazardous energy by machines types and " the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; " procedural steps for the placement, removal and transfer of lockout devices or tag out devices and the responsibility for them; and " specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tag out devices, and other energy control measures.
