29 CFR 1904.2(a)(2): If one or more of the company's establishments are classified in a non-exempt industry, the employer must keep OSHA injury and illness records for all of such establishments: a) Company-wide: An employee sustained a back injury on April 25, 2012 and the employee missed one day of work and had six days with restricted work duties. The employer did not maintain injury and illness records such as the OSHA 300 log for 2012, on or about June 12, 2012. b) Company-wide: An employee sustained an injury to the head on June 23, 2011 and the employee required one day of restricted work duties. The employer did not maintain injury and illness records such as the OSHA 300 log for 2011, on or about June 12, 2012. c) Company-wide: An employee sustained an injury to the left index finger and the employee required two days of restricted work duties. The employer did not maintain injury and illness records such as the OSHA 300 log for 2011, on or about June 12, 2012. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
