On or about June 5, 2011, Jackhammer Operation at 1401 Watertower Lane, Meridian, Idaho: During jackhammering and moving of concrete blocks, safety footwear was not utilized to protect toes from impact or compression forces. Footwear did not provide protection as prescribed in ANSI Z41.1-1967. Abatement certification is required for this item.
