29 CFR 1926.96:  The occupational foot protection or safety-toe footwear for employees did not meet the requirements and specifications in American National Standard for Men's Safety-Toe Footwear, Z41.1-1967.    On or about June 5, 2011, Jackhammer Operation at 1401 Watertower Lane, Meridian, Idaho:  During jackhammering and moving of concrete blocks, safety footwear was not utilized to protect toes from impact or compression forces.  Footwear did not provide protection as prescribed in ANSI Z41.1-1967.    Abatement certification is required for this item.

On or about June 5, 2012, Jackhammer Operation in front of 1401 Watertower Lane in Meridian, Idaho:  The employer provided dust masks for voluntary use and did not train employees in the content of Appendix D of 29 CFR 1910.134. 29 CFR 1910.1200(h)(1) (as referenced by 1926.59): Employers did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about was introduced into their work area.     On or about June 5, 2012, Jackhammer Operation at 1401 Watertower Lane in Meridian, Idaho:  The employer had not conducted hazard communication training for the hazards encountered in the workplace.  This included, but was not limited to, respirable dust containing silica during jackhammer operations.    Abatement certification is required for this item.
