29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter: a) Warehouse: : Where employees are expected to fight fires, employees were exposed to hazards through the employer not establishing or implementing an educational program to familiarize employees with the general principles of fire extinguisher operation during incipient stage fire fighting. Observed on or about 6/8/2012. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

Observed on or about 6/8/2012. b) Warehouse: Warehouse workers were exposed to struck by hazards due to the employer not training operators of Toyota Model 7FGCU25 propane powered sit down powered industrial truck as to the specific operating instructions, warnings and precautions for each type of truck/forklift operated. Observed on or about 6/8/2012. c) Warehouse: Warehouse workers were exposed to struck by hazards due to the employer not training operators of Hyster 30 Model R30XMS2 battery powered, stand-up ride on picker powered industrial truck as to the specific operating instructions, warnings and precautions for each type of truck/forklift operated. 29 CFR 1910.1200(b)(4)(iii): In work operations where employees only handle chemicals in sealed containers which are not opened under normal conditions of use, the employer did not ensure that employees were provided with information and training in accordance with 29 CFR 1910.1200(h) to the extent necessary to protect them in the event of a spill or leak of a hazardous chemical for a sealed container: a) Warehouse: Employees were exposed to chemical hazards on the jobsite do to various bags of dry product chemicals spilled in the storage areas. These products were noted to contain Crystalline Silica, Portland Cement, Nuisance Dust, sodium Carbonate, Gypsom, and Chromates. Employer did not provide employees with effective information and training on hazardous chemicals in their work area, to the extent necessary to protect them in the event of a spill or leak. Observed on or about 6/8/2012. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
