29 CFR 1910.119(f)(1)(iii): The employer's written operating procedures did not address safety and health considerations: A.) Dean Dairy Product Company, 1858 Oneida Lane, Sharpsville, PA 16150- On or about June 8, 2012, and at times prior thereto, the standard operating procedures developed for the ammonia refrigeration system did not contain specific information related to the Safety and Health Considerations relevant to the equipment and materials addressed in the procedures. In that, precautions necessary to prevent exposure, including engineering controls, administrative controls, and personal protective equipment were not were not included in procedures.

29 CFR 1910.119(j)(2): The employer did not establish written procedures to maintain the on-going integrity of process equipment: A.) Dean Dairy Product Company, 1858 Oneida Lane, Sharpsville, PA 16150 - On or about June 8, 2012, and at times prior thereto, the MI program developed for the ammonia refrigeration system did not contain specific criteria to determine when ITPM activity must increase. The companys written program stated that the test and inspection results shall be reviewed by the Plant Engineer at least annually to determine the need for any changes in frequency of tests, inspections, or preventive maintenance. No criteria for frequency changes were included.
