29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    This violation was most recently observed on June 12, 2012, at the jobsite located at 12203 Lake June Road, Balch Springs, Texas; where employees were exposed to fall hazards while standing on the top rail of the Skyjack scissor lift without the use of a personal fall arrest system.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    This violation was most recently observed on June 12, 2012, at the jobsite located at 12203 Lake June Road, Balch Springs, Texas; where an employee was exposed to fall hazards of 21-feet while decking the roof of the building without the use of a guardrail system or personal fall arrest system.
