29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system. a) New sign - On or about 11 June, 2012 - Employees were installing brackets on the inside of the sign and were exposed to a fall hazard when they were not wearing personal fall protection. Employees were exposed to a fall height of approximately 9 feet.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) Sign Base - On or about 11 June, 2012 - Employees were working around the base of the sign without the use of hardhats while another employee was removing sheet metal pieces from the top of the sign 14 feet 7 inches above them. Employees were exposed to a struck by hazard.
