29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: On or about Tuesday June 12, 2012 at the front of the steel frame building; a). The employees were observed climbing the frame of a steel fabricated mobile scaffold to access the top of the steel frame building at heights up to 12 feet 6 inches and exposed to a fall hazard.
