Violation occurred on or about 6/18/2012. b) Establishment - Door # 38: The TCM fork lift truck, identified by the employer as unit # 171, was being operated with two broken clamps that were designed to secure the propane tank in place. Violation occurred on or about 6/18/2012. c) Establishment - Door # 40: The Komatsu fork lift truck, identified by the employer as unit # 795591, was being operated with one broken clamp that was designed to secure the propane tank in place. Violation occurred on or about 6/18/2012. d) Establishment - Door # 42: The Yale fork lift truck, identified by the employer as unit # 794584, was being operated with one broken clamp that was designed to secure the propane tank in place.
