29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: On or about June 1, 2012 an employee worked in a 15 foot excavation that had near vertical walls attaching drainage piping to a catch basin without being protected from a wall collapse and subsequently being struck, trapped and/or buried by the cave-in.
